The California Healthy Kids Survey (CHKS) is an anonymous, confidential survey of school climate and safety, student wellness, and youth resiliency. The survey will be administered to students in grades three through 12. This data is used to improve the school learning environment, academic achievement, and the health and wellbeing of our youth. It includes questions on the supports provided to children; school connectedness, climate, and safety; and risk factors such as violence and substance use.

Student participation is voluntary and anonymous. Your child does not have to take the survey. No names or any other identifying information is connected to the answers except for the name of the school.
 
The CHKS is a companion tool to the California School Staff  Survey (CSSS) for staff and the California School Parent Survey (CSPS) for parents. Together, they form the California School Climate, Health, and Learning Survey (Cal-SCHLS) System.

Parent Consent

Parent Consent

The survey will be administered to students in Grades 3-12. Please keep in mind that Parent Consent is required for student participation:
  • 3rd-6th Graders: The Active Parental Consent Form in English or Spanish needs to be completed by parents/guardians to give permission for their child to take the survey. 
  • 7-12 Graders: The Passive Parental Consent Form in English or Spanish needs to be completed by parents/guardians only if they do NOT wish their student to take the survey.
